Athletic Physicals

Please see the links below for information regarding athletic physicals. You must use the GHSA Sports Physical form for all Woodland Middle School athletics. Athletic physicals are considered acceptable for one calendar year from the exam date.

As per Georgia High School Association By-Law 1.41(c) and the new State of Georgia law, the “Pre-Participation Physical Evaluation” form may be signed by a licensed Nurse Practitioner or a Physician’s Assistant provided this person has been delegated that task by an M.D. or D.O. Alterations (edits) to this copyrighted document are not permitted. Therefore, the doctor or his/her designee may print and then sign his/her (their) name on the appropriate line(s) found on page 3 and page 4 of the physical form.

Any physical marked "Not Cleared" is considered unacceptable for participation in Woodland Middle School athletics until a new physical is administered and the student-athlete is given a "cleared to participate" in the designated activity.

In addition, please review the Woodland Middle School Athletic Contract for information regarding expectations of our student-athletes.

Return to Play Act

In accordance with Georgia Law and national playing rules published by the NFHS, any athlete who exhibits signs, symptoms or behaviors consistent with a concussion shall be immediately removed from the practice or contest and shall not return to play until an appropriate health care professional has determined that no concussion has occurred. (NOTE: An appropriate heath care professional may include licensed physical (MD/DO) or another individual under the supervision of a licensed physician, such as a nurse practitioner, physician assistant, or certified athletic trainer who has received training in concussion evaluation and management.

  • a) No athlete is allowed to return to a game or practice on the same day that a concussion (a) has been diagnosed, OR(b) cannot be ruled out.

  • b) Any athlete diagnosed with a concussion shall be cleared medically by an appropriate health care professional prior to resuming participation in any future practice or contest. The formulation of a gradual return to play protocol shall be part of the medical clearance.

  • c) It is mandatory that every coach in each GHSA sport participate in a free, online course on concussion management prepared by the NFHS and available at here least every two years – beginning in 2013 – 2014.

  • d) Each school will be responsible for monitoring the participation of its coaches in the concussion management course, and shall keep a record of those who participate.

  • e) Distribute to every athlete and his/her parents/guardian an information sheet that includes: the dangers of concussion injuries, the signs/symptoms of concussion, and the concussion management protocol outlined in the by-law. This sheet must be signed by each parent/guardian of each athlete.
